All FHB Squad / Team Members have signed the following:
    

• In accepting the position of a player with the WBJSZ I agree to:

• Abide by the Code of Conduct of Football Queensland and agree not to engage in the use of crude, foul or abusive language.

• I agree not to engage in any conduct detrimental to the image of the game when on or off the field of play.

• Abide by the FHB dress code at all times.

• I agree to wear the FHB uniform with pride at all times during the any carnival as determined by the Manager.

• Treat the Coaching Personnel, Officials, my fellow team mates and spectators with respect.

• To be punctual and to wear the correct soccer attire for all trainings, camps and games.

• Attend all training sessions, camps and games organised by FHB coaches.  In the event of sickness, I will contact the Manager.

• Act in a professional manner and display good sportsmanship at all times.

• Not to use any banned substances or drugs. 

• I also agree not to smoke or consume alcohol whilst being part of the FHB squad and/ or team.

• To refrain from arguing with the Referee or Assistant Referees regarding decisions made in games.

NOTE TO PLAYERS AND PARENTS:   
It should be noted that the agreement signed by both the player and parent applies to all members of FHB Rep training squads and final teams.  Any breach thereof shall be subject to disciplinary action by the CMC with possible exclusion from the squad or team.
